CHENNAI: Often, human error is considered a reason for aviation accidents, but they are merely symptoms of a larger problem, said IndiGo Airlines fleet captain Nikhil Mathen on Tuesday.
Speaking on air safety at the weekly meeting of the Rotary Club of Madras, Captain Mathen said cultural issues like the first officer should not speak up or should use salutations like ‘sir’ or ‘madam’ while speaking to the captain and the deployment of pilots and issues like making pilots answerable to go round can all lead to human error at the time of a disaster.
